29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) 3301 Columbus Blvd., Philadelphia, PA - Maintenance Shop - Maintenance Shop - The employer did not develop hazardous energy control procedures for conducting maintenance activities, such as the replacement of hydraulic cylinder on a fifth wheel of a Yard Jockey Truck. On or about November 17, 2021, while removing a hydraulic cylinder of of the Yard Jockey, an employee suffered crushing injuries when the employees arm was caught-in-between fifth wheel and the frame of a Yard Jockey.
