29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: A.) George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania 16127, on or about August 28, 2020 and times prior thereto: Column F for Case No. 2020-62 on the 2020 OSHA 300 log provides the object/substance that directly injured or made the person ill, but does not provide the injury or illness, or the part of the body affected. B.) George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania 16127, on or about August 28, 2020 and times prior thereto: Column F for Case No. 2020-61 on the 2020 OSHA 300 log provides the part of the body affected, but does not provide the injury or illness, or the object/substance that directly injured or made the person ill. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
