29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy might be supplied to the particular lines and equipment were de-energized / inoperable (i.e. open) and then did not ensure that such means were tagged to indicate that employees were at work: a) Washington Electric Cooperative, Inc. located at 36200 Paw Paw Rd. Lower Salem, OH 45715: On and preceding March 31, 2020, the employer did not ensure that sources of electric energy which supplied lines being worked on were de-energized and then did not ensure that the system was tagged. A crew consisting of three lineman were performing work on a section of electrical distribution line with the line not properly de-energized/tagged, tested and no ground was installed on the system the crew was performing work on. The following condition exposed the employees to electrical hazards.
