29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: a. Terra Dining Hall, Kitchen - The employer did not provide employees with training on the hazards of mixing bleach (sodium hypochlorite) with incompatible cleaning chemicals, such as, acids (eg. Lime-A-way) and cleaning compounds that contained ammonia. An employee became ill after she had inhaled a mixture of bleach, Lime-A-way, Spic & Span, and a degreaser.
