29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on equipment (such as compressors, mixers, screens, or pumps used for concrete and masonry construction activities) where the accidental start-up of the equipment was not eliminated by use of lock out/tag out procedures: On or about June 24, 2019, at 1494 Northlake Way, Palme Beach, Florida, 33480, the employer failed to develop and implement lock-out / tag-out procedures prior to allowing an employee to clean the inside of a cement pump hopper, thus exposing the employee to an amputation hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
