29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Employees engaged in production activities in the die-cutting production area were exposed to struck-by, caught-between and amputation hazards in that the Preco Die-Cutting Press machine was not guarded to prevent point of operation injuries. Employees were exposed to struck-by, caught in between and amputation hazards. Employees engaged in production activities were exposed to injury including amputation. The Preco Die-Cutting Press machine was not guarded to prevent access to the point of operation.
