29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Mid-States Packaging, Inc., located at 12163 State Route 274, Lewistown, OH 43333: On or about April 15, 2019, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, the 100 Ton Freeman Schwabe Automatic Press (Model # BP100-5979H; Serial # FHH1009). Employees did not control all hazardous energy sources such as, but not limited to, electrical before conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to conducting work.
