29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits could be energized. On or about May 12, 2018, in the attic of a residential dwelling where employees performing heating, ventilation and air conditioning (HVAC) work were exposed to electrical shocks from energized circuits that were not rendered inoperative and tagged.
