29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: a) Greenville Overlook, Lot #88 - On or about April 27, 2018, the employer failed to identify and evaluate respiratory hazards to employees operating fuel powered equipment in an enclosed space. An employee operating a fuel powered Ryobi 2700 pressure washer in an unventilated basement became unconscious and was hospitalized. The employee was exposed to 8-hour Time Weighted Average (TWA) of 196.3 parts per million (ppm) Carbon Monoxide (CO), which is 3.93 times greater than the OSHA Permissible Exposure Limit (PEL) of 50 ppm CO. The employee's exposure was determined by using post-exposure carboxyhemoglobin (COHb) measurement.
