29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from caught between hazards associated with inadequate machine guarding. The most recent occurrence of this was found at the facility located at 1204 Jones St. Omaha, NE where the employer has employees opening the Plexiglas door and reaching into the Bettcher Dyna-Form meat press to fill the meat log form or to remove the finished meat logs from the forms; with the Plexiglas door interlock not functioning properly, resulting in an employee having four fingers amputated.
