29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed. On or about May 3, 2017, and times prior thereto, employer exposed employees to hazardous energy when they did not remove all employees from the work area to ensure that all employees have been safely positioned or removed when testing the lockout procedures for the XV484 valve.
