29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) SDAC Kingsman Building: On or about 07/09/2020, a maintenance employee trained only as an affective employee attempted to remove the motor pulley and measure the shaft of return fan number 11 was not trained as an authorized employee. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
