29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting face piece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): As referenced in 1910.1052(g)(2) a) Onyx Restoration Ltd. d.b.a. Alliance Restoration. Employees working at 215 Park Ave. South New York, NY 10003, were exposed to the hazards of Methylene Chloride (MC) contained in Zip-Guard Premium Paint & Finish Remover. The employees were provided and required to wear half face tight fitting respirators with air filtering cartridges. The employer did not ensure the employees required to wear the respirator passed a respirator fit test before using the respirator. The condition was noted at or around September 11, 2018. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
