29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached: A). On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY Two employees were each using a separate personal fall arrest system while working from a 3 and 12 pitched roof. The roof brackets for each personal fall arrest system were fastened to the roof using only 3 of a possible 32 fasteners.
