29 CFR 1926.1053(b)(15):Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. On or about December 9, 2021 at the job site located at 1670 Firestone Parkway Akron, Ohio 44301; during the removal of lead at a commercial facility, the employer did not ensure portable ladder were inspected prior to use. An employee utilized a portable ladder that was missing the rubber foot pad that would have prevented the ladder from slipping out while under load. The overlanding surface was approximately 10 feet above the ground.
