29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about March 9, 2020, the employer did not ensure employees performing servicing and maintaining th Velcro Package Machine No. 1. followed the established energy control procedures by; ensuring the machine was turned off [per 1910.147(d)(2) requirements], isolating the machine from the energy source(s) [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to servicing and troubleshooting the machine.
