29 CFR 1910.109(e)(5):"Warning required." Before a blast is fired, the employer did not require that a loud warning signal be given by the person in charge, who did not make certain that all surplus explosives were in a safe place, all persons and vehicles were at a safe distance or under sufficient cover, and that an adequate warning was given. a) Economizer 2 Top - Prior to the warning signal the person in charge did not make certain that all persons were at safe distance or under sufficient cover, on or about September 29, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
