29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a) At The Track, Gulf Shores, Alabama: On or about August 26, 2020 and a times prior; the employer failed to enter a recordable injury on the OSHA 300 Log within 7 days involving an employee fracturing a toe on June 17, 2020. b) At The Track, Gulf Shores, Alabama: On or about August 26, 2020 and a times prior; the employer failed to enter a recordable injury on the OSHA 300 Log within 7 days involving an employee placed on lite duty for two days on June 17, 2020. c) At The Track, Gulf Shores, Alabama: On or about August 26, 2020 and a times prior; the employer failed to enter a recordable injury on the OSHA 300 Log within 7 days involving an employee who suffered a sprained ankle and missed one day of work on July 29, 2020. d) At The Track, Gulf Shores, Alabama: On or about August 26, 2020 and a times prior; the employer failed to enter a recordable injury on the OSHA 300 Log within 7 days involving an employee who suffered a sprained ankle and missed 2 days of work on July 31, 2020. e) At The Track, Gulf Shores, Alabama: On or about August 26, 2020 and a times prior; the employer failed to enter a recordable injury on the OSHA 300 Log within 7 days involving an employee cutting their foot requiring inpatient hospitalization on August 5, 2020.
