29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 11045 Old Hwy 31, Spanish Fort, AL 36527: On August 20, 2020, and at times prior, the employer exposed employees to workplace hazardous, such as but not limited to cave in, struck-by, and crushed-by hazards, while working in and near trenches and excavations, in that the employer has not provided trench safety training.
