29 CFR 1910.147(c)(5)(ii)(A)(1): Lockout and tagout devices were not durable; they were not capable of withstanding the environment to which they were exposed for the maximum period of time that exposure was expected: A.) Milwaukee Forge, LLC as a successor to Mercer Forge Corporation/Hot Trim Press -- Lindell 2000 trim press (TP03) - On or about 7/16/2020, and at times prior thereto, the top of the trim box was only being support by a 2 x2 wood block when an employee's body parts were inside the machine performing activities that included but were not limited to securing and removing the trim box. The wood block failed resulting in the top portion of the trim box striking an employees fingers resulting in serious injuries. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
