29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Location - 850 Main Street, Wilmington, MA, Building G: On or about 05/16/2020, an employee performed servicing and/or maintenance work involving the removal of the barrier guard and exposure to the danger area created by the moving parts of the RU 2/28-80 Braider, a cord and plug connected piece of electrical equipment, and the cord was not unplugged and maintained under the exclusive control of the employee, nor was the equipment's hazardous energy otherwise controlled.
