29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) 230 South Duncan Street, Luck, WI; At the Cantek surface planer, on or about December 11, 2019, an employee performing service work was exposed to hazardous energy associated with moving machine parts as residual energy was not relieved or otherwise rendered safe [(d)(5)(i)] and verification of isolation [(d)(6)] was not performed following the application of lockout device(s).
