29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: A) Buck Brothers LLC, on or about October 22, 2019, at the address of 2727 Avondale Avenue, Toledo, Ohio: The employer failed to instruct all employees in the recognition and avoidance of hazards commonly seen in the Asphalt Paving industry. An untrained employee operating the Maulin Converyor Feeder Asphalt Paver Model 1750-C during refueling was exposed to crushed-by hazards.
