Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Folding Department, RO3 F.L. Smithe Champion Rollamatic Folding Machine. On or about February 6, 2020, employees who performed servicing and maintenance tasks such as clearing jams in the Seal Flap Folder area of the machine were exposed to amputation, laceration and crushing hazards to hands and fingers from the ingoing nip points between moving rollers. Employees were not required to control hazardous energy sources in the following sequence when clearing jams: 1) turn off and shut down the machine, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy sources, 3) affix lockout or tagout devices to each energy isolating device, 4) relieve, disconnect, restrain or otherwise render safe all residual/stored energy sources, 5) verify that isolation and deenergization of the machines or equipment have been accomplished. Alternative protective measures, such as machine guarding, were not adequate. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
