29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On September or about 10, 2019 located near Orion station area employees who performed work activities associated with work underneath or ensuring section(s) of track with cantilevered attached catwalk load rigged by a rough terrain forklifts elevated forks and sling attached to the catwalks guardrails while employees were underneath the cantilevered catwalk torqueing bolts or when sling(s) are released during the process maintained structural stability of the load were not trained on the applicable steel erection regulations, regulations associated with processes of maintaining structural stability of loads or regulations associated with working underneath loads exposing employees to potential crushing hazards by the overturning structural steel section of track with cantilevered attached catwalk.
