29 CFR 1910.147(d)(4): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Location:2101 91st St North Bergen, NJ 07047 - production floor a) On or about 06/13/2019 a worker was injured when reaching into Tunnel #2 washing machine to remove linen caught inside. During the procedure the machine remained in operation with the linen striking and injuring the employee's hand. The employee failed to follow the machine specific energy control procedure in order to properly isolate the machine. Condition noted on or about 06/13/2019
