29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: On or about March 27, 2019, and times prior thereto, the employer did not include "Loading Slurry Lime Trailer and Trailer Clean-out" in the Site Hazard Assessment and PPE Selection document dated 05/25/15 and revised on 01/16/19. Employees were potentially exposed to severe skin and eye irritation and burns when loading and rinsing the hydrated lime slurry trailers.
