29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) Science & Technology Building, Penthouse Utility Room, on or about 4/17/19: An authorized employee was performing maintenance activities on the cooling tower fans and did not personally and physically lockout the equipment at the source. The authorized employee relied on another employee working in the Emergency Management Systems Control Room to turn the fans off by computer. The unexpected start up of the fan resulted in a finger tip amputation.
