29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: a) Plasma Cutting Area: The L shaped base hooks (chain sling) were not securely attached to the plasma cutter slag tray which permitted the tray to slip from the base hooks and strike the operator causing severe injuries, on or about January 23, 2018.
