29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components: a) Andersen's Sales & Salvage, Inc. 1490 E. 8th Street, Greeley, CO 80631: On and before October 12, 2017 the employer did not remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components. The employer was replacing the rotor, hammers, and pin protectors on the MegaShredder model 88x112 M-Series hammer mill. The locking pin that held the rotor in position was removed several times during this process to allow the rotor to rotate to a balanced position. When the locking pin was removed, and the rotor was rotating, the employer did not ensure that employees were removed from the working platforms located adjacent to the rotor. An employee was injured when he was caught in-between the rotating rotor and a working platform. This condition resulted in exposure to rotating parts, ingoing nip points, and amputation hazards.
