29 CFR 1915.89(q)(1): The employer shall conduct an audit of the lockout/tags-plus program and procedures currently in use at least annually to ensure that the procedures and the requirements of this section are being followed and to correct any deficiencies. 29 CFR 1915.89(q)(3)(i): A review of the written lockout/tags-plus program and procedures; On and about June 30, 2017, at Fincantieri Marine Group, LLC. 605 N. 3rd. St. Sturgeon Bay, WI, an annual audit of abrasive blasting machines specific LOTO/tags-plus procedure did not ensure the procedures were incompliance with the OSHA Standard. The original procedure and an updated, post-incident procedure did not include methods to notify affected employees of equipment being locked out.
