29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Flatwoods Job Corps, 2803 Dungannon Rd, Coeburn, VA 24230: On March 6, 2017, and times prior thereto, the employer did not guard points of operation on a Piranha Ironworker at the shearing station and at the coping-notching station, exposing employees and contracted instructors to amputation and crushed-by hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
