29 CFR 1918.24(i)(1)(ii): Ladders with broken or split side rails were used or were not tagged as unusable or removed from the vessel: a) IMTT-Bayonne, Ladder from Inspected Barge John Blanche to the Pier 1 West, Bayonne, NJ. The 12 foot wood single ladder that extended from the barge to the wood bumper of the pier had a 20 inch long split in one of its rails along with other smaller splits along the rails exposing workers to falls if a ladder failure would occur. Violation occurred on or about 10/16/2015.
