29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: The employer does not establish and implement a written respiratory protection program with worksite specific procedures. This violation occurred on August 31, 2013 and at times prior thereto, at a jobsite located at 3009 Binkley Avenue, Dallas, Texas; where the employer did not establish and implement a written respiratory protection program with worksite specific procedures for employees wearing supplied-air respirators: Instance a: Procedures to ensure adequate air quality for atmosphere supplying respirators; Instance b: Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations; Instance c: Procedures for regularly evaluating the effectiveness of the program.
