29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: a. On or about August 30, 2023, the employer did not ensure that procedures for clearing the flop gate (Procedure FP2-21) were specific in addressing all elements concerning hazardous energy control such as, but not limited to, specific elements concerning the purpose of the procedures; steps for controlling hazardous energy; identify the magnitude of hazardous energy; location of the isolation point; and methods to be used.
