29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: a) De-barker area: On or about December 7, 2022, employees were exposed to caught-in and struck-by hazards when the employer did not ensure that a written control of hazardous energy procedure was documented and utilized by employees to lock-out equipment such as, but limited to, the de-barker and conveyors before entering the access-restricted areas where unguarded equipment was located.
