29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. On or about October 30, 2021, at the above addressed jobsite, an employee, performing maintenance operations on a residential lawn sprinkler system, while adjacent to and/or standing in the Kankakee River, was not provided with or required to wear a life jacket or buoyant work vest, thereby exposing the employee to the danger of drowning.
