29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 21, 2021, in the production area, the employer failed to provide effective guarding to protect employees working at or near the Jumbo Straw Extruding Machines #1. The guarding device for area between crimping wheel and straw guide with ingoing rollers, the area underneath the takeaway conveyor with exposed moving conveyor belts, and the area underneath the handwheel with ingoing rollers did not prevent employees from operating the machine while in the zone(s) of danger during the extruding process. Employees were thereby exposed to the hazards associated with unguarded ingoing nip points and rotating parts during the operating cycles. b) On or about October 21, 2021, in the production area, the employer failed to provide effective guarding to protect employees working at or near the Jumbo Straw Extruding Machines #2.
