29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following 29 CFR 1910.147(c)(4)(ii)(A)-(D): (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and, (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. 60 slitter, Yoder tension stand: on or about April 5, 2021, the control of hazardous energy procedures "Lock Out/Tag Out Document#: P406-A" and "Lock Out / Tag Out Procedures Hydraulic Unit - 60" Slitter Lockout Disconnect #5" were developed for preventive maintenance and repair work performed by maintenance personnel. These procedures did not contain the required specific information for use by non-maintenance personnel assigned to perform carpet inspections, adjustments, and changes (setup) which exposed their hands to a moving ram caught-in hazard between the upper and lower boards (clamp) when equipment was not de-energized in correct sequence to ensure safety. These procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the tension stand in order to safely de-energize the equipment and control the hazardous hydraulic energy. The procedure also lacked specific steps to relieve hazardous hydraulic energy prior to equipment access and specific verification steps were not prescribed to determine the effectiveness of the energy control measures. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
