29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about April 14, 2021, the ladder used in the tortilla making area was not inspected before use or as necessary to identify any visible defects. This condition exposed the employees to a fall hazard.
