Location - Throughout 15 Marion St, Lynn, MA 01905 On or about 1/20/2021, operators of powered industrial trucks did not receive a three year operator performance evaluation to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely. An employee was involved in an incident with a powered industrial truck that resulted in an injury.
