29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A. On or about August 12, 2019, Production: The employer had not developed machine-specific lockout procedures; documented training for authorized, affected and other employees; and annual reviews of those written procedures to address the unexpected energizing, startup or release of stored energy for employees servicing or maintaining shop equipment, including honing machines. An employee operating the Sunnen ML-3500 honing machine suffered a crushing injury to two fingers on the left hand while reaching behind a guard to adjust the oil flow while the mandrill was in motion. The equipment was not shut down or locked out prior to the adjustment being made.
