29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On, or about, June 17, 2019, the following mechanical power presses were not locked out when tool punches/inserts were changed. The stations were not protected by safeguarding such as a light curtain, when employees accessed the tooling: A. Press 50, Bliss partial revolution press, station accessed through hinged plexiglass door at back of press. B. Press 332, a V&O partial revolution press, station accessed through the side of the guard. C. Press 334, a Walsh partial revolution press, station accessed through the side of the guard.
