Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: On or about July 3, 2014 and in times prior thereto a Miller derrick belt was in use and was equipped with tool rings that could be mistaken for positioning attachments. The waist belt was for work positioning only and the tool rings were not intended to support a person. The D ring in the upper back section of the harness was the only ring designed for fall protection and the employer did not ensure the employee hooked it to the fall protection anchorage. The fall hazard was located in the derrick of Drilling Rig #4.
