29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not provided, used, and/or maintained. On or about February 19, 2014 and at times prior thereto, in the Melt Shop, the employer did not provide a shield or barrier wall between the Ladleman's Table and the overhead crane carrying a ladle with 36 - 38 tons of molten steel.
