29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.: (a) Facility, 19-20 Battery, Near the #7 Quench Tower: On or about January 31, 2014, two employees were working next to an open trench. This trench was not equipped with either a cover or a guardrail. (b) Facility, Inside and Outside of Quench Tower 7a: On or about March 20, 2014, employees were working next to open trenches inside and outside of the #7a quench tower. These trenches were not equipped with either covers or guardrails.
