Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about April 5, 2012 - at the above addressed jobsite, the employer allowed employees to conduct lifting operations without properly designing and specifying the procedures for welding lifting lugs to a lifted steel plate, thereby exposing the employees to struck by hazards. Due to this hazard, an employee was struck by the lifted steel plate after the improper weld failed. One feasible and acceptable method to abate this hazard, among other means, would be for the contractor to fabricate and inspect welding in accordance with ANSI-AWS American Welding Society D1.1 Structural Welding Code-Steel.
