Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee in that the employee were exposed to a fall hazard: a) Starflyer ride - An employee was exposed to a fall hazard when using a 6-foot Protecta lanyard with the one-foot Avanti rail runner while climbing the mast ladder. The 6-foot lanyard used in conjunction with the rail runner would allow the employee to hit the ground, or a lower level/equipment, before the fall was arrested. As per the Avanti Fall Protection System's User's Manual and Installation Instructions, "never adapt, extend, or in any way change this [fall protection] system [consisting of the runner, the shock absorber, and the carabiner.]" Abatement note: among other methods, one feasible and acceptable means of abatement is to comply with Avanti Fall Protection System User's Manual and Installation Instructions, i.e. "never adapt, extend, or in any way change this system."
