Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Well Site near Kelton, TX, Rig #134, Well: Neeley #505: On 7/29/08, and times prior thereto, employees were exposed to potential struck by hazards while they were performing job tasks on the rig floor as mud was circulating under high pressure through circulating pipe which was not secured to prevent movement should the line fail. American Petroleum Institute Recommended Practice RP54 Paragraph 9.13.3 states that: "Each section of a high pressure rigid line should be secured using appropriate means to prevent movement should the line fail. Flexible high pressure hoses should be properly secured to prevent unsafe movement. Other suspended hydraulic, air, or electrical lines should be appropriately secured." Note: Abatement certification and supporting documentation are required for this item.
