Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and caught-by hazards: On or about August 28th, employees were exposed amputation and caught-by hazards in that an operator was not stationed at the drawworks controls while employees were using the right hand side cathead to hoist a new boom line on Rig #2 at SEC 16, TWP 25, R 9 in Langdon, Kansas. Among other methods, one feasible and acceptable method to correct this hazard are to follow Section 9.5 "Catheads and Lines Powered By The Cathead" of API RP 54 Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations. Third Edition, August 1999, Reaffirmed, March 2007. Section 9.5.6. "The drawworks control shall be attended while a manually operated cathead is in use." 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
